Silvercare Solutions can help.  Our Continence Care Solution puts a Nurse Practitioner/Continence Specialist in your facility on a regular basis to provide continence care to your residents.  The Nurse Practitioner /Continence Specialist provides specialized clinical services that can improve patient quality of life, reduce risk of co-morbidity and support nursing staff in their endeavors to provide outstanding quality care.  Working within the scope of practice as an Advanced Practice Nurse, the Continence Specialist collaborates with the Medical Director and nursing staff to achieve the best outcome possible for each resident while helping facilities meet CMS Surveyor Guidelines. 

Cost of Services
The Nurse Practitioner/Continence Specialist works with you in the capacity of independent consultant and is an Independent Medicare Provider.  Therefore, costs for services rendered and care provided to the resident are billed directly to Medicare Part B or appropriate private insurance carrier by the Continence Specialist.  This is important, as your facility incurs no additional staffing cost for these services.

 

 


  The Continence Care Solution is composed of three parts:
  • The Continence Specialist begins by providing an in-depth evaluation using advanced nursing skills.  As part of this evaluation, a comprehensive review of the medical history is performed to identify factors that may be contributing to the incontinence. 

  • A physical examination, which includes evaluation of genito-urinary tissues: the vagina, rectum and pelvis, is performed.  Alterations in structure and function are assessed.  Measurement of post-void residual volume, urinalysis and bedside cystometry are used as appropriate.  The Continence Specialist is responsible for compiling the information gathered to determine the type of incontinence and related diagnoses. 

  • Treatment options are then recommended with individualized
    plans of care predicated on this careful evaluation of each resident. Treatment strategies may include vaginal estrogen replacement, anticholinergic therapy, pessary placement, pelvic muscle rehabilitation with biofeedback, toileting, bowel management and skin care.  Documentation of the consultation that is included in the medical record is provided upon completion of the visit with evidence-based continence outcomes identified using our proprietary Treatment Value Report.
